Title: Price effects in health care utilization - regression discontinuity evidence

We assess effects of patient cost sharing on physician visits among young adults, exploiting a natural experiment in a regression discontinuity design. In the setting of a Swedish region, health care services are universally provided free-of-charge for children, but patients over 20 years are charged a copayment of SEK100 (Euro10) for a physician's visit. The analysis is performed using a longitudinal register data set over two years of 72,000 individuals aged 18-22 years, studying visits to general practitioner as the outcome. Results show a small but significant decrease (6.2 percent) in visits to general practitioner caused by the introduction of copayment at age 20, corresponding to an arc elasticity of -0.03, hence very inelastic demand. In subgroup analyses, we find females are twice as price sensitive as males, and a clear distinction in the effect with respect to income, where the quarter with lowest income is six times or more price sensitive than the quarter with highest incomes.
The results indicate that there is little evidence of moral hazard in the demand for visits to general practitioner and that relatively low copayments serve as an efficient instrument to add to budget revenue. Despite this, the discrepancies found in the copayment effect based on gender and income suggest copayments contribute to inequalities in health care use, possibly increasing inequalities in population health.
